FAMILY HEALTH TEAM PROGRAMS

Interdisciplinary programs run by Nursing, Dietitians, Clinical Pharmacists, and Social Work.

  • SMOKING CESSATION PROGRAM

Smoking Treatment for Ontario Patients (STOP), is a program designed by the Centre for Addiction and Mental Health and funded by the Ministry of Health and Long-term Care, to help patients quit smoking.  The program provides free Nicotine Replacement Therapy (patches, gum, lozenges, inhalers) to patients who smoke and are registered with the North York FHT.  One-on-one counseling sessions are also provided to patients who enroll in the program.

  • MINDFULNESS-BASED CBT

Mindfulness is for patients that need help living ‘in the moment’ and being able to feel grounded during times of stress or high emotions. MBCT combines Meditation with CBT Psychotherapy to help you effectively manage depression or anxiety.

  • CBT - COGNITIVE BEHAVIOR THERAPY

Cognitive Behavioral Therapy is for patients over 17 that are struggling with feeling sad, anxious, or who are dealing with a major life transition. There are three groups within this program; CBT for Anxiety and Depression, CBT for insomnia, and CBT for Self Esteem & Assertiveness.

Complete take-home readings and exercises to help you understand more about why you might act and feel the way you do

Enjoy a safe place to talk about feeling stressed, worried, or anxious

Start setting small, attainable goals to help you feel better

  • HUNGER FOR HEALTH

How often have you heard yourself say, “I know what to eat, but I just can’t seem to do it.”? This six week, interactive group program explores “why” we struggle with our eating.

 

What will I gain from attending? 

Learn new strategies to manage problematic eating

Learn to create a more positive relationship with food

  • DIABETES EDUCATION PROGRAM

The North York Family Health Team Diabetes Education Program is made up of a nurse and dietitian, both certified diabetes educators.

 

We offer the following services for adults:

 

Group Classes: 

Pre-diabetes, Type 2 diabetes

 

Individual Classes: 

Pre-diabetes, Type 2 diabetes, Glucometer Teaching, Post-gestational diabetes and Insulin Start/Titration

  • CENTRE FOR COMPLEX DIABETES AT NORTH YORK GENERAL (BRANSON SITE)

While most patients usually manage quite well with standard levels of care, the CCDC provides comprehensive care for patients with complex diabetes needs. Care is provided over a shorter term, is patient-centered, goal specific, and allows for frequent follow-up.

CCDC Patients typically experience one or more of the following:

Unmanaged diabetes complications

Comorbidities which impact glycemic control

Barriers in accessing health care

Recurrent hospitalization or Emergency Room visits

Complexities requiring case management
