Instructions for appointments with Dr. Sari Herman

Please bring a valid health card to your appointment.

 

If you do not speak fluent English, please bring someone with you to interpret.

 

If possible, please refrain from taking any allergy medication or antihistamines for 72 hours prior to your appointment.  All other medications should be taken as normal.

 

Please arrive 15 minutes prior to your appointment to allow time for parking and registration.  Parking is a flat rate of $4 for 2 hours.  The parking machines are located inside the lobby of our building.  You will be required to enter your license plate number.  It is not necessary to leave anything in your car.

 

We value our patient's time.  Unfortunately, due to the nature of the type of appointments Dr. Herman provides, we cannot always predict the length of appointments.  Dr. Herman is thorough and takes her time with each patient.  Please be prepared to wait for up to 1 1/2 hours to be seen. 

 

We require 2 business days to cancel or reschedule.  Failure to do so will result in a $100 missed appointment fee, which must be paid prior to rescheduling.
